CHW program is open for referrals. If you are working with a family/individual that meets the eligibility below please, send an email to chwreferrals@elprograma.org with the participant’s full name, best contact phone number, and the city they reside in. Once on the list, they will receive a call from a CHW within 24 hours.

• CHWs provide wraparound services to families currently in quarantine or isolation

• CHWs are able to and depending on need, assist with rental assistance and/or utilities
